Buffy - Her voiceover speech to Dawn as she dives into the vortex.

Doctor Who - Rose with her father when he figures out what he has to do and he tells Rose and Jackie. I cry every damn time I watch that in the episode.

Angel - Fred dying in Wes' arms

CSI: Miami - Alexx performing the autopsy on Speed

Dresden Files (book) - Harry and Thomas seeing their mother in the soulgaze.

Those are what usually do it for me. My most heartbreaking moments are usually from classic stuff and involve either platonic or romantic love gone wrong, like when Ebenezer Scrooge lets his love walk away or when Long John Silver tells Jim Hawkins he really did like him before he saved him, or when Athos confronts Milady Dewinter.

Exactly! I love those moments. I also love the family parts but then I'm a sucker for Hallmark commercials, especially during Christmas. :D
